You may have an annular tear or fissure, or your condition may have progressed to Leaky Disc Syndrome. These conditions are often misdiagnosed or underdiagnosed.
Most likely, you have had an MRI of your spine. While excellent at assessing the spinal discs to see if they’re bulging, ruptured, or pressing on the spinal cord or nerves, an MRI is NOT sensitive enough to detect annular tears.
The only way to diagnose an annular tear is by a test called an Annulargram™. An Annulargram is a pain-free and precise x-ray-guided technique invented by Dr. Kevin Pauza. Using very small needles, a small amount of X-ray contrast is injected into each disc’s annulus fibrosus to identify tears and leaks.
Unfortunately, many cases remain improperly diagnosed because the connection between back pain and annular tears has only recently begun to be fully recognized within the medical community.
Let’s back up for a moment and briefly explain the anatomy of spinal discs.
The outer collagenous rings called the annulus fibrosus
The inner gel-like substance called the nucleus pulposus
Over time (from daily activities, age, or injuries), your disc (the outer ring area) can become torn. This causes weakness in the disc. As these tears progress, the inner gel-like substance from the nucleus pulposus can leak out onto adjacent spinal nerves. Your body assumes this leakage is a “foreign substance” and goes into an immune response. Creating pain and inflammation.
Lower back problems including degenerative disc disease, herniated disc (also called slipped, ruptured, or bulging disc), sciatica, and others are the result of annular tears.
Does your pain move around? Meaning sometimes you feel it on the left, then on the right, or in the center of your lower back. You could have multiple tears in different discs.
Annular tears and fissures can’t repair themselves. When left untreated, they could progress to other conditions like a herniated disc, bulging disc, or ruptured disc. Traditional back surgery and suturing (stitches) cannot repair a tear.
